In your arms and legs are structures called compartments. Groups of muscles and nerves and blood vessels wrapped in fascia. If the pressure in one of those increases, if that part gets injured and swells up, or bleeding into the compartment, it cuts off the circulation to the rest of your limb. If that lasts too long, you lose a hand or an entire leg. To keep that from happening, they just cut a hole in you.
I had it in my leg, not very badly, but it's why I had to spend so long in the hospital. The day after I saw you, the doctor punched a hole in the back of my leg to let all the stuff drain out. They let me stay awake and watch. It was pretty cool.
